Teaching Parents About Car Safety

Hello, I'm Dr. Flaura Koplin Winston. I'm a general pediatrician, a biomechanical engineer, and I co-direct The Center for Injury Research and Prevention at The Children's Hospital of Philadelphia. On March 21, 2011, the American Academy of Pediatrics released new guidelines on child passenger safety, based on up-to-date information. In another Medscape video, I reviewed these recommendations. In this video, I'm going to give you practical information on how to relay this information to families and answer their questions. I'm going to focus on 2 controversial recommendations that caused a lot of buzz. The first one is to keep kids rear-facing until at least age 2 years. The other is to keep kids in booster seats longer, past age 8 years, until they reach 4 foot 9 inches, which might be between the ages of 8 and 12 years."

Severe Blisters and an Erythematous Rash on a Nursing Home Resident: Analyze This: Image of the Week

A bedridden 85-year-old woman with advanced multi-infarct dementia is presented from a nursing home for evaluation of a rash on her abdomen with a possible skin infection at her gastrostomy tube (G-tube) site.
The nursing-home staff first noticed the rash 2 days ago. It apparently began after levofloxacin therapy was started for a urinary tract infection. Because skin creases and the umbilicus were involved, a topical antifungal was started, but no improvement was noted. The patient has no fever or rigors.
On physical examination, the patient smiles when spoken to and has good ocular tracking, but she cannot meaningfully verbalize and follows only the simplest of commands. Her vital signs are within normal limits, and she appears to be in no distress. Other than baseline right-hemiparesis, examination yields unremarkable results except for the skin findings."

Medscape: Hello Dr. Volkow. What was your reaction to the WHO report concluding that electromagnetic fields from cell phones are 'possibly carcinogenic?' Do you believe that the available data support this conclusion?

Dr. Volkow: I think that the report was justified on the basis of results that are inconsistent but which cannot be ignored. It seems prudent in this situation -- in which there are some results [linking cell phone use with malignancy] -- to be cautionary. I think that is why they came up with this recommendation.

It wasn't strong evidence, which the authors of The Lancet paper discuss. However, they couldn't just dismiss and ignore the findings.

Medscape: I found it interesting that the INTERPHONE study showed that in all exposure groups except that with the highest cell phone exposure, there was actually a reduced or equal incidence of glioma compared with those who'd never used a cell phone. What do you make of this finding?"
